Quick answer
All-on-4 is a full-arch treatment in which four implants, usually two upright at the front and two tilted at the back, carry a fixed bridge of teeth. At Elite Dental Clinic in Hyderabad, it is planned from a 3D scan to check that your bone can hold the implants.
01 Overview
What is All-on-4 and how does it work?
All-on-4 is a specific way of restoring a whole jaw of teeth on four implants. Two implants are placed upright at the front of the jaw. Two more are placed at the back, tilted outward at an angle, so that they sit in the bone that is available in front of nerves or sinuses and can use a longer implant. A fixed bridge of teeth is then attached to the four. It is one of several full-arch plans, see full mouth dental implants for the others.
A reason for tilting the back implants is to avoid bone grafts in many patients, because the implants are placed in bone that is already there. It also lets the bridge be supported at a point closer to the back, so less of it hangs unsupported.
A systematic review in 2013 of 13 studies counted 4,804 implants, of which 74 failed, and 74 percent of those failures happened in the first 12 months. The authors found promising short-term results, but said that data beyond five years were scarce and that the main complication of the prosthesis was fracture of the all-acrylic bridge. Average bone loss was about 1.3 mm at 36 months.1
The same plan is sometimes done with six implants, see All-on-6 dental implants. The choice depends on your bone, your bite and your own priorities.
In short, All-on-4 carries a full arch on four implants, and the evidence is good in the short term and thin beyond five years.
Where the back of the jaw is short, the bridge can extend slightly past the last implant, and this overhang is called a cantilever.
02 Candidacy
Am I suitable for All-on-4 in Kondapur?
Suitability depends on your bone, gums, bite and health. A scan decides. The two lists below describe the general pattern.
Who it often suits
- Most or all teeth in a jaw missing or beyond saving
- Dentures that move or rub and make eating hard
- Enough bone at the front, or room to place tilted implants at the back
- Good general health and the ability to clean under a fixed bridge
Needs caution or a different plan
- Very thin bone at the front, where four implants may not be enough or a graft is needed
- Strong clenching or grinding, which can break an acrylic bridge
- Active gum disease or infection, which is treated first2
- Smoking or poorly controlled diabetes, which raise the risk of failure
All-on-4 suits patients with enough front bone who can clean under the bridge and who do not grind heavily.
03 Options
What are the choices within the All-on-4 plan in Kondapur?
Even within All-on-4, there are choices to make. The dentist explains them from your scan.
All-on-4 with tilted implants at the back
The standard approach: two straight implants at the front and two tilted at the back, carrying a fixed bridge. It aims to use available bone and avoid grafts.1
Standard plan
Same-day temporary teeth after All-on-4 surgery
In suitable cases a temporary fixed bridge is attached soon after the implants, so you leave with teeth. This is called immediate loading. The 2013 review included 1,201 prostheses fitted within 48 hours of surgery.1 It is not suitable for everyone.
Within days
Acrylic bridge or a stronger final bridge
The first bridge is usually acrylic. A final bridge may be made of acrylic over a metal frame or of zirconia. The main prosthetic problem in the review was fracture of the all-acrylic bridge.1
Final stage
If your bone allows six implants, see All-on-6 dental implants. For a removable alternative, see dentures.
| Compared on | All-on-4 | Removable denture |
|---|---|---|
| Held by | Four implants, fixed | Gum, with or without adhesive |
| Surgery | Yes | No |
| Cleaning | Special brushes under the bridge | Take out and clean |
| Chewing | Firm, like fixed teeth | Can move |
All-on-4 gives fixed teeth with surgery, and a denture avoids surgery but can move.
04 Procedure
How is All-on-4 done, step by step?
The treatment takes several weeks to months, depending on whether temporary teeth are fitted and how you heal. The steps below are what usually happens.
Planning on a scan, surgery in one visit, healing, and a final bridge.
Examination and 3D scan to plan the All-on-4 implants
The dentist examines your mouth and takes a 3D scan to measure the bone, nerve positions and sinuses. The positions and tilt of the four implants are planned and shown to you.
Treating gum disease and removing remaining teeth
Gum disease and infection are treated, see periodontal treatment. Teeth that cannot be saved are removed, often at the surgery visit, see tooth extraction.
Placing four implants, two of them tilted
Under local anaesthetic, with sedation if advised, the two front implants are placed upright, and the two back implants are placed at an angle. The bone is smoothed if needed. The gum is closed.
Fitting a temporary bridge with immediate loading
Where the implants are steady enough, a temporary fixed bridge is attached to them. If not, a soft denture is worn while the bone heals.
Healing and bonding of the implants
The bone grows around the implants over a few months. You eat soft food and keep the area clean. Reviews check the gums and the bridge.
Fitting the final bridge on the four implants
When the implants are firm, impressions or scans are taken and the final bridge is made, tried in, adjusted for your bite and fixed to the implants. A night guard may be advised.
05 Cost
What decides the cost of All-on-4 in Kondapur, Hyderabad?
Treatment is planned for one person, not sold as a product, so the cost follows your evaluation.
Every mouth needs a different plan, so Elite Dental Clinic does not quote a cost before an evaluation. You receive a written plan that lists each stage before any treatment starts.
What the evaluation looks at
- Whether one or both jaws are treated
- The implant system and the type of final bridge
- Whether teeth need removing or bone needs grafting
- Whether temporary teeth are fitted on the day
What you decide with the dentist
- The scans, sedation and number of visits
- Treatment of gum disease or decay first
- A night guard and maintenance visits afterwards
Your general health matters too, including diabetes, smoking and any medicines that affect bone healing.
The cost follows the plan, and the plan follows the evaluation.
06 Aftercare
What is it like to recover from All-on-4?
Recovery takes a few days for the surgery and a few months for the bone. The steps below help it go well.
After surgery
Expect swelling, soreness and bruising for several days, and some oozing on the first day. Use cold packs, rest with your head raised and take only the medicines prescribed.
Eating with a temporary bridge
Eat soft, cool food and cut it small. Do not bite into hard or sticky food, even if the temporary teeth feel firm. The bone needs undisturbed time to bond with the implants.
Cleaning under the bridge
A fixed bridge needs special cleaning: interdental brushes, a water flosser and superfloss under the bridge. Poor cleaning is the main cause of gum and bone problems around implants.
Long-term care
Do not smoke. Attend maintenance visits, wear a night guard if you grind and expect that the acrylic teeth may need repair or replacing over time.1
When to call us
Call the clinic the same day if bleeding does not stop with pressure, swelling worsens after the third day, you have a fever or pus, or a tooth on the bridge breaks or loosens. If swelling spreads to the face or neck, or you have trouble breathing or swallowing, go to the nearest hospital emergency department.
Soft food while healing, special cleaning under the bridge and a night guard protect the result.
07 Limits
When is All-on-4 the wrong choice?
All-on-4 is usually delayed or replaced by another plan in these situations:
- Very thin or soft bone at the front of the jaw, where four implants may not be enough.
- Heavy grinding or clenching that is not controlled, which can break an acrylic bridge.1
- Active gum disease or infection that has not been controlled.2
- Smoking or poorly controlled diabetes that the patient cannot change.
- Radiotherapy to the jaw or medicines that affect bone, which need a doctor's opinion first.
- A patient who cannot clean under a fixed bridge or attend maintenance visits.
For plans using more implants, see All-on-6 dental implants and full mouth dental implants. For one missing tooth, see single tooth implant.
Risks and complications
The risks include infection, pain and swelling, nerve or sinus problems, implants that fail to bond, loosening or breaking of the bridge, and inflammation of the gum and bone around the implants that can cause bone loss.
In the 2013 review, 74 of 4,804 implants failed, most within a year, and the main prosthetic complication was fracture of the all-acrylic bridge.1 A later review of complete-arch prostheses found that having fewer than five implants did not lead to worse survival than more.3
The authors noted that evidence beyond five years is limited.1 No one can promise that a full-arch bridge will last for any number of years.
All-on-4 works well in the short term, and its long-term record is still limited.
08 Your visit
What happens at your first All-on-4 appointment in Kondapur?
At the first visit the dentist listens to your concerns, examines your teeth, gums and bite and looks at X-rays or a scan. You are told whether All-on-4 is likely to suit you and what the other options are, including six implants and dentures. You receive a written plan that lists each stage before any treatment starts.
Bring any earlier X-rays and dentures, and a list of your medicines. Tell the dentist about diabetes, smoking, heart conditions, grinding and any treatment of the jaw.
The team speaks English, Telugu and Hindi. You can bring a family member. You are under no obligation to go ahead with treatment.
The first visit ends with a scan-based plan, not a commitment.

All-on-4 is a clever way to use four implants where the bone allows, and for the right patient it can mean fixed teeth with less surgery and no bone graft. It is not a shortcut for everyone. I would rather show you the scan, explain the trade-offs and tell you if six implants or a different plan would suit you better.
Dr. Revathi Indukuri
Dr. Revathi Indukuri reviewed this page as a dentist at the clinic. The clinic's team board lists a prosthodontist and implantologist, a periodontist and oral and maxillofacial surgeons, which are the roles a full-arch plan draws on, from the implants to the final teeth.
Surgery and teeth planned together
The team board lists an implantologist, a periodontist, a prosthodontist and oral surgeons, so the surgery and the bridge are planned as one.
Honest about four versus six
You are shown the scan and told whether four implants are enough or six would suit you better.
A written plan before treatment
You receive a written plan before any treatment starts, so you know what comes next.
Gum health first
Gum disease around the teeth is treated before implants, because it raises the risk of failure.
A joined-up team, honesty about four versus six and a written plan are what this clinic stands behind.

13 Questions
All-on-4 questions from patients in Kondapur
What is All-on-4?
It is a way of restoring a full arch of teeth on four implants. Two are placed upright at the front and two are tilted at the back, and a fixed bridge of teeth is attached to them. It is used when most or all teeth in a jaw are missing or cannot be saved.
Why are two implants tilted?
Tilting the back implants lets them sit in the available bone, in front of nerves or sinuses, and use a longer implant. In many patients it avoids a bone graft and supports the bridge closer to the back.
Can I get teeth on the same day?
Sometimes. If the implants are steady enough, a temporary fixed bridge can be fitted within a day or two. The 2013 review included prostheses fitted within 48 hours. If the implants are not steady, a soft denture is worn while the bone heals.
How long do All-on-4 implants last?
No one can promise a number of years. In the 2013 review, 74 of 4,804 implants failed, mostly in the first year, and the main complication was fracture of the acrylic bridge. Data beyond five years were limited. Cleaning and maintenance affect the result.
Is All-on-4 better than All-on-6?
Not necessarily. A review of complete-arch prostheses found no relationship between the number of implants and survival. Six implants spread the load more and may suit patients with enough bone. The dentist explains which fits you.
Is the surgery painful?
It is done under local anaesthetic, with sedation if advised, so you should not feel pain during it. Swelling, soreness and bruising are common for several days and are managed with the medicines the dentist prescribes.
How is the cost of All-on-4 decided?
The cost is given after the evaluation and scan. Whether one or both jaws are treated, the implant system, the type of final bridge, any grafts or tooth removal, and the number of visits all change the plan. You receive a written plan before treatment starts.
Can I have All-on-4 if I have very little bone?
Possibly, but it depends on where the bone is missing. Tilted implants can use bone that is there and avoid a graft in some cases. If bone is very thin, a graft or a different plan may be needed. A scan shows what is possible.
14 Second opinion
Thinking about All-on-4 or another full-arch plan?
Bring your X-rays, any scans and the advice you have had so far. The dentist will go through them with you and explain whether four implants would be enough for your bone, and when six would suit better. Call or send a WhatsApp message to book. The evaluation is the first step, and you are under no obligation to go ahead with treatment.
